How To Register

Before proceeding make sure to have the following information ready in order to make registration procedures run smoothly for your team!

1. All 3-5 members of your teams information such as: address, full name, age and contact info.

2. If you are under the age of 18, please have a legal parent/guardian sign for all players to be eligible for play.

3. UNDERSTAND THE RULES & DATES.

*Open registration to ALL teams will be accepted as of 6/1/2010. However, teams and participants that are NOT from the FSUSD will be put on a list in order received and will have priority when registration for OUTSIDE COMPETITORS opens.

6/1/2010 - OPEN REGISTRATION TO ALL COMPETITORS!

7/16/2010 - REGISTRATION CLOSES. ANY LAST MINUTE TEAM(s) WILL HAVE TO REGISTER ON A "FIRST COME FIRST SERVE" SIGN-UP LIST AT THE BEGINNING OF THE EVENT ON JULY 31st,2010.

1. DOWNLOAD ONLINE PDF REGISTRATION FORM.

2. MAKE SURE ALL REQUIRED INFORMATION IS FILLED OUT AND EMAIL, HAND-IN, OR FAX APPLICATIONS TO THE FOLLOWING: (more hand-in outlets will be available the closer it gets to the deadline)

3. REGISTRATION PAYMENT METHODS
(all checks must be made payable to: FHS Athletics and can be sent to
SOAP FHS ATHLETICS 340-2 Travis Blvd. PMB #340, Fairfield, CA 94585 )

b. Cash and check can be made at MUNCHIES COFFEE. no credit cards
c. Mail checks to address above
  
  *IF your choose to email or fax registration form: Payments are due with 5 days of submission. Failure to do so will result in forfeit of your registered tournament placement. Hand-In registering teams will be required to pay upon submission. (please see REGISTRATION FORM for pricing)

4. THATS IT! After registering your team you will receive a confirmation email stating your acceptance to the tournament and will be updated on any information pertaining to the event.
